Many opponents of BIG programs believe that receiving guaranteed subsistence income would act as a strong disincentive to work. In contrast, various areas of empirical research in psychology suggest that a BIG would not lead to meaningful reductions in work. To test these competing predictions, a comprehensive review of BIG outcome studies reporting data on adult labor responses was conducted. The results indicate that 93 % of reported outcomes support the prediction of no meaningful work reductions when the criterion for (...) support is set at less than a 5 % decrease in either average hours worked per week or the rate of labor participation. Overall, these results indicate that adult labor responses would show no substantial impact following a BIG intervention. (shrink)

Much innovation has taken place in the development of medical schools and licensure exam processes across the African continent. Still, little attention has been paid to education that enables the multidisciplinary, critical thinking needed to understand and help shape the larger social systems in which health care is delivered. Although more than half of medical schools in Canada, the United Kingdom, and the United States offer at least one medical humanities course, this is less common in Africa. We report on (...) the “liberal arts approach” to medical curricula undertaken by the University of Global Health Equity beginning in 2019. The first six-month semester of the curriculum, called Foundations in Social Medicine, includes courses in critical thinking and communication, African history and global political economy, medical anthropology and social medicine, psychology and health, gender and social justice, information technology and health, and community-based training. Additionally, an inquiry-based pedagogy with relatively small classes is featured within an overall institutional culture that emphasizes health equity. We identify key competencies for physicians interested in pursuing global health equity and how such competencies relate to liberal arts integration into the African medical school curriculum and pedagogical approach. We conclude with a call for a research agenda that can better evaluate the impact of such innovations on physicians’ education and subsequent practices. (shrink)

The letter deals with the establishing and development of philosophical education in the Department of Philosophy of Addis Ababa University. It explores the focus of philosophical education since its inception, unravelling a trajectory that began with a few courses and has now reached a level where a PhD in teaching is offered and important research undertaken. It explains how certain philosophical issues have influenced the evolution of philosophical education in Ethiopia. The dynamics of philosophical education in Ethiopia is constituted by (...) the debate on the nature and task of philosophy. This highlights a healthy and fruitful debate by students of philosophy pointing out that philosophy can be contextualised to enlighten us on social and moral questions without, of course, forgetting the traditional and main branches of philosophy. Some of the African universities were established just over half a century ago, the overwhelming majority of them coming into being after independence. They came into being largely not on the basis of the desire of the African peoples but rather to serve a purpose related to colonialism. Even when this was not the purpose, the way they were established and organized, i. e. irrelevant curricula biased against the local knowledge and culture and an equally biased faculty made higher education (...) in Africa into a white elephant. Lack of resources and irrelevance of curricula did not enable the African university to play the role that is expected of it. The developments since the 1980s show the dependence of African universities. The so‐called reforms of the World Bank and Co. are meant to reduce African universities to vocational schools. Taking these into account the paper argues that the African university must try to recreate itself by overcoming extraversion and restructuring itself in such a way that it is ready to use universal knowledge without undermining indigenous knowledge. Rapid Ethical Assessment is an approach used to design context tailored consent process for voluntary participation of participants in research including human subjects. There is, however, limited evidence on the design of ethical assessment in studies targeting cancer patients in Ethiopia. REA was conducted to explore factors that influence the informed consent process among female cancer patients recruited for longitudinal research from Addis Ababa Population-based Cancer Registry. Qualitative study employing rapid ethnographic approach was conducted from May–July, 2017, at the Tikur (...) Anbessa Specialized Referral Hospital. In-depth and key informants’ interviews were conducted among purposively selected 16 participants. Regular de-briefings among the study team helped to identify emerging themes and ensure saturation. Interviews and debriefings were tape recorded in Amharic, and transcribed and translated to English. Coding of the transcripts was facilitated by use of NVivo software. Thematic analysis was employed to respond to the initial questions and interpret findings. Perceived barriers to voluntary study participation included lack of reporting back study results of previous studies, the decision making status of women, hopelessness or fatigue in the patients, shyness of the women, data collectors approach to the patient, and patient’s time constraints. Most of the patients preferred oral over written consent and face-to-face interview over telephone interview. Provision of detail information about the study, using short and understandable tool, competent, compassionate and respectful enumerators of the same gender were suggested to assure participation. Due to the perceived severity, the use of the term “cancer” was associated with fear and anxiety. Alternatively, uses of phrases like “breast or cervical illness/disease” were suggested during patient interviews. Voluntary participation is not straight forward but affected by different factors. Using competent, compassionate and respectful enumerators, short and precise questioning tools to limit the time of the interview could improve voluntary participation. Moreover, careful consideration of the patients and families concept of the disease such as wording and information has to be taken into account. This assessment helped in improving the consent process of the ongoing project on breast and cervical cancer patients. (shrink)

The Hatata Inquiries are two extraordinary texts of African philosophy composed in Ethiopia in the 1600s. Written in the ancient African language of Geʿez (Classical Ethiopic), these explorations of meaning and reason are deeply considered works of rhetoric. They advocate for women's rights and rail against slavery. They offer ontological proofs for God and question biblical commands while delighting in the language of Psalms. They advise on right living. They put reason above belief, desire above asceticism, love above sectarianism, and (...) the natural world above the human. They explore the nature of being as well as the nature of knowledge, the human, ethics, and the human relation with the divine. They are remarkable examples of something many assume doesn't exist: early written African thought. This accessible English translation of the Hatata Inquiries, along with extensive footnotes documenting the cultural and historical context and the work's many textual allusions, enables all to read it and scholars to teach with it. The Hatata Inquiries are essential to understanding the global history of philosophy, being among the early works of rational philosophy. The book includes a translation by Ralph Lee with Mehari Worku and Wendy Laura Belcher of the Hatata Zara Yaqob and the Hatata Walda Heywat. The appendices by Jeremy R. Brown provide information on the scribal interventions in and the differences between the manuscripts of the two Hatatas. The book also includes a map, chronology, summary of the translation principles, and a discussion of the authorship debate about the Hatata Inquiries. (shrink)

A critical element in building stronger health systems involves strengthening good governance to build capacity for transparent and fair health planning and priority setting. Over the past 20 years, the ethical framework Accountability for Reasonableness has been a prominent conceptual guide in strengthening fair and legitimate processes of health decision-making. While many of the principles embedded within the framework are congruent with Western conceptualizations of what constitutes procedural fairness, there is a paucity in the literature that captures the degree of (...) resonance between these principles and the views of decision makers from non-Western settings; particularly in Africa, where many countries have only recently, within the last 20-30 years, become more democratic. This paper contributes to the ethics literature by examining how Ethiopian decision makers conceptualize fair and legitimate health decision-making, and reflects on the degree of conceptual resonance between these views and the principles embedded in A4R. A qualitative case study approach from three districts in Ethiopia was undertaken. Fifty-eight decision makers from district, regional, zonal, and national levels were interviewed to describe their conceptualization of fairness and legitimacy in the district health planning process. Findings revealed that Ethiopians have a broad conception of fairness and legitimacy that while congruent with procedural justice, also aligned with principles of distributive and organizational justice. Researchers and practitioners seeking to strengthen procedural fairness in health priority setting must therefore recognize the significance of other philosophical dimensions influencing how fairness and legitimacy of health decision-making are constructed within the Ethiopian setting. BackgroundRespecting patients’ confidentiality is an ethical and legal responsibility for health professionals and the cornerstone of care excellence. This study aims to assess health professionals’ knowledge, attitudes, and associated factors towards patients’ confidentiality in a resource-limited setting.MethodsInstitutional based cross-sectional study was conducted among 423 health professionals. Stratified sampling methods were used to select the participants, and a structured self-administer questionnaire was used for data collection. The data was entered using Epi-data version 4.6 and analyzed using SPSS, version 25. Bi-variable and (...) multivariable binary logistic regression analyses were used to measure the association between the dependent and independent variables. Odds ratio with 95% confidence intervals and P value was calculated to determine the strength of association and to evaluate statistical significance.ResultOut of 410 participants, about 59.8% with [95% CI (54.8–68.8%)] and 49.5% with [95% CI (44.5–54.5%)] had good knowledge and favorable attitude towards patents confidentiality respectively. Being male (AOR = 1.63, 95% CI [1.03–2.59]), taking training on medical ethics (AOR = 1.73, 95% CI = [1.11–2.70]), facing ethical dilemmas (AOR = 3.07, 95% CI [1.07–8.79]) were significantly associated factors for health professional knowledge towards patients’ confidentiality. Likewise, taking training on medical ethics (AOR = 2.30, 95% CI [1.42–3.72]), having direct contact with the patients (AOR = 3.06, 95% CI [1.12–8.34]), visiting more patient (AOR = 4.38, 95% CI [2.46–7.80]), and facing ethical dilemma (AOR = 3.56, 95% CI [1.23–10.26]) were significant factors associated with attitude of health professionals towards patient confidentiality.ConclusionThe findings of this study revealed that health professionals have a limited attitude towards patient confidentiality but have relatively good knowledge. Providing a continuing medical ethics training package for health workers before joining the hospital and in between the working time could be recommended to enhance health professionals’ knowledge and attitude towards patient confidentiality. This essay proposes African-based ethical solutions to profound human problems and a working African model to address those problems. The model promotes sustainability through advanced agroecological and information communication technologies. The essay’s first section reviews the ethical ground of that model in the work of the Senegalese scholar, Cheikh Anta Diop. The essay’s second section examines an applied African model for translating African ethical speculation into practice. Deeply immersed in European and African ethics, Godfrey Nzamujo developed the Songhaï Centers to (...) solve the problem of rural poverty in seventeen African countries. Harnessing advanced technologies within a holistic agroecological ecosystem, Nzamujo’s villages furnish education spanning the fields of ethics, information communication technology, microbiology, international development, and mechanical, electrical, civil and biological engineering in a community-based and centered development enterprise. The essay proposes a global consortium of ecovillages based on Nzamujo’s model. The final section explores funding methods for the consortium. The Ḥatäta Zärʾa Yaʿǝqob and Ḥatäta Wäldä Həywät are two remarkable works of philosophy that were until recently virtually unknown to philosophers outside Ethiopia. The first is a philosophical autobiography narrated by the eponymous Zärʾa Yaʿǝqob, a scholar from Aksum in northern Ethiopia, exiled from his home country and forced to take refuge in a mountain cave, where he develops a philosophical system that encompasses a metaphysics of creation, an analysis of the relation between the divine and the human, an (...) account of both epistemology and ethics grounded in the faculty of ləbbuna or ‘reason’, and a strident critique of contemporary social norms. The second text is a much longer companion treatise written by his disciple Wäldä Həywät, and develops the arguments and ideas of Zärʾa Yaʿǝqob into a systematic social ethics. These works, essentially unexamined by philosophers before the past decade, have enjoyed a certain notoriety within the field of Ethiopian-Semitic philology, on account of a century-long controversy concerning their authorship.
